Structured framework supporting families before, during, and after their time at the Family Assessment Centre.

At Nucleus Horizons, we recognise that transitions can be one of the most challenging times for families. Clear, well-planned transitions are essential to safeguarding children, supporting parents, and ensuring that any progress made during assessment can be sustained. Our Clear Transition Plans are designed to support families before, during, and after their time at the Family Assessment Centre, providing continuity, clarity, and stability throughout each stage of the journey. This child-focused approach helps families move through each stage with the right support, planning, and safeguarding measures in place.
Clear Transition Plans are commissioned by local authorities and the courts and form a structured framework that supports families at key points of change. This includes entry into residential assessment, movement between assessment models, and transition back into the community or onward planning. All transition planning is undertaken in line with the Children Act 1989 and the Working Together to Safeguard Children (2020) framework. Our approach is child-focused, strengths-based, and proportionate to risk. Transitions are carefully planned, reviewed, and adjusted in partnership with families and professionals to ensure children’s safety and emotional wellbeing remain central.
Our transition planning service provides structured, practical support across all stages of a family’s involvement with Nucleus Horizons. This includes careful preparation, ongoing review, and coordinated family support to help promote safe outcomes for children and greater stability for parents.
