What We Do

Clear Transition Plans are commissioned by local authorities and the courts and form a structured framework that supports families at key points of change. This includes entry into residential assessment movement between assessment models, and transition back into the community or onward planning.


All transition planning is undertaken in line with the Children Act 1989 and the Working Together to Safeguard Children (2020) framework. Our approach is child-focused, strengths-based, and proportionate to risk.


Transitions are carefully planned, reviewed, and adjusted in partnership with families and professionals to ensure children’s safety and emotional wellbeing remain central.

Our Clear Transition Planning Service

Our transition planning service provides structured, practical support across all stages of a family’s involvement with Nucleus Horizons. This includes careful preparation, ongoing review, and coordinated family support to help promote safe outcomes for children and greater stability for parents.



Before coming to stay with us:



Before admission, we work closely with the placing authority to ensure families are fully prepared. This includes:



  • Pre-placement planning meetings
  • Review of risks, supervision levels, and assessment aims
  • Clear explanations provided to parents about what to expect
  • Practical preparation for moving into the centre
  • Coordination with relevant professionals and services



Where possible, families are offered the opportunity to visit the centre in advance to support a smooth and informed transition. This early planning helps reduce uncertainty and supports a safer start to the placement.



When with us:



Throughout the assessment, transition planning remains an active and ongoing process rather than a single event. This includes:



  • Regular review meetings with families and professionals
  • Ongoing assessment of readiness for change or step-down
  • Gradual preparation for community living where appropriate
  • Support to build routines, confidence, and independence
  • Planning for next steps based on assessment progress



This ensures that families are not faced with sudden or unsupported changes at the end of their placement. It also supports care planning, family stability, and better long-term outcomes for children.



After leaving us:



Following completion of the residential assessment, we support families to transition safely and confidently into their next stage. This may include:



  • Step-down support into the community
  • Continued community-based assessment or support
  • Practical transition planning and handover to local services
  • Support with maintaining routines and learning
  • Liaison with professionals to ensure continuity of care



Where required, we can provide short-term post-placement support to help stabilise placements and promote sustainability. This continued support helps families maintain progress and strengthens continuity of care after leaving the centre.
